Swan Theatre Company Member / Patron Subscription Renewal 2010 

Your subscription to the Swan Theatre runs out on 31^st March 2010.  The cost of renewal is £12.50 for members and £7.50 for patrons. 

If you wish to renew your subscription for the forthcoming year (1^st April 2010 – 31^st March 2011) please: 

1.       Complete the membership form.

2.       Tick your areas of interest so our database can be updated.

THEN EITHER

3.   Return this form with cheque payments by 31^st March (cheques should be made payable to The Swan Theatre).

             OR

4.   If you currently pay, or wish to start paying by, standing order please complete the standing order form, and return both forms to the Membership Secretary at the Swan Theatre by 31^st March. 

IF you are a tax payer please also complete the gift aid declaration form to enable us, as a charity, to reclaim the basic rate tax amount.
